If the patients do not have knowledge or resources for telehealth communication, the implementation of this plan will not be feasible. 

Measuring Outcomes

It is hard to measure the impact of any proposed plan if the outcomes are not documented. Thus it is very important that a proper plan is set in place for measuring the outcomes for the proposed interventions. 

Outcome Criteria that will be Evaluated

The proposed health promotion plan makes use of self-management plan and the use of telehealth tools. The health progress in the patients is one of the prime outcome that we need to evaluate. This can be analyzed through the blood pressure levels of the patients since it is tracked in the telehealth application. 

Communication Plan

The proposed health promotion plan can be effectively applied for the promotion of better health in the community. Telehealth tools allow for an enhanced communication between the patients and the healthcare providers. 

Community Stakeholders

The stakeholders in this health promotion plan include firstly the patients and their families. The other stakeholders include the nursing staff and medical health care professionals. The IT team who are in charge of managing telehealth system for the hospital are also a stake holder.

The use of the proposed health promotion plan will allow the patients of hypertension to experience more autonomy over their health. Reduced number of hypertension cases will also mean a large chunk of money and medical resources which are spent on the treatment of hypertension can be saved. 

Resource Alignment to Health Improvement Plan

The use of self-management plan relies on imparting education to the patients about their condition and the risks associated with the health care issue they are suffering from. For the use telehealth tools the electronic gadget and internet facility are the resources which will be required.
