Introduction

NR443 W6 SBIRT Screening, Brief Intervention, and Referral to Treatment Presentation

Alcoholism has been believed to be a genetic addiction but there are other factors in people lives that lead them to this disease such as the individual, environment, social status, and culture.

“Slightly more than half (51.8%) of Americans aged 12 or older reported being current drinkers of alcohol in the 2011 survey, making for 133.4 million current drinkers in 2011” (Nies,2019 ).

According to the Department of Health in the Bronx County 12.5% are binge drinkers which rates them the highest out of the county group rankings.

Addiction/Problem Description

“Each year, approximately 115 million visits are made to hospital emergency departments.1 An estimated 15% to 20% of these patients come to the emergency department with alcohol use problems.2-7 This number increases to 25% to 50% in trauma center patients.5-8 In fact, alcohol is the single greatest contributor to injury in the United States,9 with an economic impact of more than $185 billion each year in personal and societal costs” {Desy, 2010}.

Alcoholism can lead to depression, medical problems, firodrugIn the Bronx, there are a lot of bars with steady occupants and liquor stores that open in the morning.  This causes drunken Many residents have lost their jobs, family, and homes due to alcohol.

Utilize one scholarly reference (not assigned in this course) to describe this problem/addiction. Discuss reasons why this may be a problem in your community. Cite all sources.

In “ The SBIRT Interview: Alcohol Screening” video I watched on YouTube the nurse interviewed the patient in a primary care setting.

She was non confrontational and to the non judgmental to the  patient who in turn was very open and honest with her.

She asked open ended questions which let him speak about his alcohol usage and come to the conclusion that he may have a problem.

He was receptive to the fact that alcohol may have been the reason why he has hurt himself and others around him.  He was then interested in seeking intervention with outside help.

SBIRT Description

“Substance use remains a leading cause of preventable death globally. A model of intervention known as screening, brief intervention, and referral to treatment (SBIRT) was developed decades ago to facilitate time- and resource-sensitive interventions in acute care and outpatient settings. SBIRT, which includes a psychosocial intervention incorporating the principles of motivational interviewing, has been shown to be effective in reducing alcohol consumption and consequences in unhealthy drinkers both in primary care and emergency department settings” (Bernstein, 2017).

Community Resources

Two community resources I found in my community was free AA meetings which are offered all around the city for free in different settings from churches to community halls that offer free support to anyone who either abuse alcohol or has an alcohol addiction.  It’s a meeting with other people who also have issues surrounded by alcohol and its an anonymous and open environment to express your feelings and concerns with out judgement.

There are many other resources in the Bronx for alcoholism that are covered by most insurances. They offer detox and counseling programs for out and inpatients.

For example Samaritan Day top Village offers a treatment program that address not only the mental disorder but the physical as well.  They receive support services that will help them see what maybe causing their addiction and ways to overcome it,

Conclusion and Future Implications for Nursing

Alcoholism is a very dangerous addiction that can not only affect the individual but also those around them

It should be treated as a disease and treated at the first warning signs

Primary care should include assessment of this disease in order to prevent harm to oneself and others

SBIRT can help identify they problem whether it be by an initial assessment or by a situation caused by alcohol use/abuse.

Intervention practices can be utilize at the first sign of the problem.   If minor,  the person can attend AA meetings and if it is someone with bigger issues it can be an out or in patient setting treatment program.
