Intervention Presentation and Capstone Video Reflection
Hello and welcome to my video reflection on intervention presentation and capstone project. My name is Julian Nkem and I will be presenting an EBP strategy and intervention for my capstone project patient. This intervention is for the health improvement and have quality of life improvement for hypertension patients as general as well. This is an audio video presentation which will explain about the role of nurse’s education in management of hypertension as well as the use of telehealth and telemedicine use for providing management program provided by American Heart Association (AHA) and National health safety committee (NHS).
This project was directed on hypertension patient named John, who was having a worsen health condition due to multiple mismanagement scenarios by nurses as well as lack of education on self-management of the hypertension and quality care. The practicum was conducted on December 5, 2021 at a public healthcare centre. The total practicum hours are approximately 12 and were followed by all ethical guidelines and considerations.
The patient had a serious complication of hypertension and hyperglycaemia and was on an edge for a stroke attack. He was a smoker and previous alcoholic and were having epilepsy episodes from 4 years approximate. He had visited the hospital for almost 4 times during the last 1 month and his condition was worsening despite of his visits due to lack of self-management education.
The nurses also had managed his condition very poorly as he was given the wrong dose of beta blockers 2 times due to which his condition became worse. Despite a normal EF, he was having a clinical heart failure and was at the risk of death if not treated well on time. He doesn’t have any technological knowledge and didn’t have any private vehicle which were some extra barriers in his treatment.

I will be explaining evidence-based strategies for the treatment of hypertension as well as use of telehealth. Moreover, I will be describing the role of nurses and how they can be educated properly to reduce hospital mismanagement.
Evidence and Peer-Reviewed Literature to Plan and Implement a Capstone Project
I will be implementing the use of telemedicine and telehealth technology interventions to increase the quality of hypertension management. Moreover, nursing education will also be enhanced in terms of use of technological interventions and hypertension management.
Telemedicine (or telehealth) is a novel method to patient management that combines different kinds of information communications technologies (ICT) to remotely give care, discussion, patient practice, particular care and public health, and to remotely monitor patients’ parameters. I t is a potential tool for increasing access to treatment, empowering people, changing their attitudes and habits, and eventually improving their medical problems (Pellegrini et al., 2020).
Because hypertension is the most frequent and significant major risk factor for cardiac disorders globally, it is an excellent target for telehealth, particularly telemonitoring. While telehealth has been demonstrated to enhance hypertension control when opposed to older treatment, its role in managing patients is unclear (Pellegrini et al., 2020). Although most regulations mention it in the perspective that would exclude shrouded high blood pressure, there are currently no available suggestions upon that role of telemedicine in overall primary care, excluding the 2017 HTN Clinical Practice Guidelines, that either suggest that telemonitoring techniques can be saving time and resources to therapies shown to lower the blood pressure in older adults (Omboni et al., 2020).
In most cases, patients and caregivers communicate in real time or on a regular basis when communicating about health issues. Illustrations of the first kind of contact include those that take place during live audio-video conference between a healthcare practitioner and the patient, as well as those that take place during real-time transfer of physiological parameters from the healthcare center to an acute medical center. Remote interpreting of earlier supplied material, email message or Texting, mobile applications, or digital educational programme are instances of sequential engagements.

How Health Policy Influenced the Planning and Implementation of Capstone Project
Telehealth alleviates the burden of the nursing workforce by providing patients with better accessibility to experts; caregivers may effectively contribute to patient care & happiness without all the other “unnecessary bureaucracy” of typical on-site consultations, resulting in dramatically enhanced effectiveness. In addition to helping their telemedicine clients, nurses need continue to employ comprehension, ability to think critically, and clinical judgement in the same way that do in formal roles (Yatabe et al., 2018).
Hypertension necessitates diagnostic testing, and it is a condition that demands early detection. There are several tests involved that cost a lot of money, but hypertension insurance covers them under the Affordable Care Act. The ACA has lowered the number of uninsured individuals to historically low levels and enabled more people, particularly low-income people and people of color, to receive health care services. Doctors may practise online before and beyond typical working hours, eventually helping more patients, thanks to the ease of telehealth (Yatabe et al., 2018). Whether it’s retired doctors who wish to continue practising, doctors who have stopped practising due to family obligations, or doctors who simply want to broaden the area of their practise, technology can assist enlarge the pool of accessible primary care physicians (Parolisi, 2020).
Capstone Project Outcomes
By implementing the proposed medication and intervention strategies for the above-mentioned patient, capstone outcomes were measured. The knowledge of improved telehealth and telemedicine increased the overall hypertension management quality and the care level of patient by nurses. Telehealth also reduced the transportation issues of patient. Education on self-management care also increased the quality of care of patient and drastically changed the condition of patient. The patient cardiac condition became normal and hi blood pressure controlled in the range of 120/80 mmHg.
Personal and Professional Growth as RN to BSN
Personal growth is a lifetime process. It is a method of assessing our abilities and attributes, considering our life objectives, and setting goals to fulfil and maximise our potential. Personal growth is pushing oneself to learn and master new talents. Personal and professional development of overlapping responsibilities for leadership and management because it encompasses the procurement of additional training and education to remain efficient in producing interest in sustainable and learning using evidence-based practise. It assists nurses and midwives in staying current with their training so that they may provide the best possible care to patients. It also allows their co-workers and management know how they’re performing and whether they require any assistance in maintaining safe and successful practises (Pellegrini et al., 2020).
This project helped me in growing as a nurse form RN to BSN as by analysing the mistakes of nurses in this scenario, I found the role of nurses in medication errors and I will avoid these errors onwards too and will use the same interventions for the treatment of patients at my organization and will avoid the same errors that were made by the nurses and will improve the overall healthcare condition of patient. The role of BSN will help me to in cooperate the multifaceted solutions to healthcare industry by using the common resources of health care organization (Yatabe et al., 2018).
