Introduction
In the health care industry, medication errors continue to affect patients, health care workers, and organizations as a whole, according to recent research findings. It is possible for a patient’s death to be caused by pharmaceutical errors. This might be due to a human error (Gorgich, Barfroshan, Ghoreishi and Yaghoobi, 2016; 2015). As a result of inadvertently entering erroneous data into a prescription medication order, technical issues may occur.
In today’s healthcare marketplace, stressors such as heavy patient loads, increasing demands on nurses and other direct care providers, and system-wide financial strain can lead to a wide variety of miscommunications and tensions among members of the healthcare team. As a result, a focus on enhancing interprofessional collaboration is much needed to help minimize undesirable events, and improve teamwork, communication, and most importantly – improve patient outcomes.

Objective and Issue
The goal will be to find a four-hour window during which the loss of access to the EMR would have the least impact on patient care activities. It’s a great chance to observe interprofessional teamwork in action while also learning how to identify multidisciplinary issues and fix them using the PDSA cycle.

Role of different Stake Holders
IT Manager will manage the update and security patching of the EMR.
Nursing Body will be responsible for the training of nurses for manual handling of the patient records’
Clinical Manager will handle the allocation of radiologist in every department for the manual test evaluations and lab results.
Administrative Manager will be available all time to run smoothly all above mentioned duties assigned to everyone.
Health Care Informatics Analyst will be responsible for linkage of all stack holders to find the appropriate time and efficiently took all the steps

PDSA Cycle
PDSA stands for plan-do-study-act. PDSA is a basic quality improvement process that allows an organization to test and analyse a change on a small scale to determine whether it is improving a process or making it more efficient.
The PDSA framework covers four phases. It allows organizations to plan to test a change in a process (plan), to carry out that test by making changes in that process (do), to observe and analyze, through measurements, the effects of that change (study), and then to decide how to react to that first test (act).

“Plan” Phase of PDSA
All stake holders will meet with each other to plan the down time.
During these meetings the IT team leaders will explain the need and reason for the down time.
Other stakeholders will share critical patient care activities that will occur in their areas during a 24-hour period.
The stakeholders will explore the pros/cons of a variety of days/times for the EMR down time
Proposed time is 2:00 AM to 6:00 AM

Reason for Proposed Time
This selection was made for the following reasons:
Allow end of day billing transactions to be completed.
Permit the phlebotomy team to begin their morning rounds on time.
Historically low volume of emergency department visits at the specific time.
A radiologist was available to be on site to read imaging studies.
Nursing unit staffing was acceptable.

“Do” Phase of PDSA Cycle
Nurses and practitioners will resort to their down-time procedures.
They will request, document, and monitor patient care activities manually.
the IT team will reboot 57 servers, apply 17 security patches to the software.
New Operating System of EMR will be installed
The updates will be tested to make sure there are no negative impact on the EMR.

Evaluation of Plan
After the Act Phase of the PDSA Cycle, Evaluation will be done for following expected results of the Plan:
Efficacy of the proposed time of 2:00 AM. to 6:00 AM will be checked
Efficiency of Nursing staff in manually organizing the records documents will be assessed .
Downtime procedures will be reviewed with all nursing staff according to the results of the PDSA act phase .
It will be checked whether additional phlebotomy staff will be needed or not to help with first round lab draws in order to minimize delays in lab result processing.
