SAFETY IMPROVEMENT PLAN
Current scenario is that a medication error was made on a medical surgical unit in a rural hospital. This error was made because the unit being busy and lack of communication between two registered nurses.
A)Plan -We will be addressing how to avoid errors like this from occurring by going back to basics and examining the six medication rights.
We will be forming groups of two and preforming an activity.

The importance of this activity is for our organization to improve patient safety by reducing medication administration errors and thusly reducing adverse effects.
Improving patient care also improves nursing performance by reducing anxiety, boost confidence, protect nurse licensure, improve organization satisfaction and reduce costs

AUDIENCE’S ROLE
Everyone will perform a mock trial of medication administration. Everyone will Break off into groups of two and perform this activity that exercises a proper medication administration that would occur while working on an acute unit in the hospital where a witness is needed.
Evaluations will be preformed to assess the pros and cons of the activity, errors will be addressed.
Opinions and suggestions will then be freely expressed on how to improve medication administration.

NEW PRACTICE AND SKILLS PRACTICE
New processes and skills can be for both RNs to make sure that that all 6 medication administration rights have been performed.
Little note cards can be printed and be put on mac cart, in medication room to remind nurses of foundational skills
State Laws and Facility Policies should be placed where medications are accessed

SOLICITED FEEDBACK
Take a poll or fill out survey of what RNs feel will be most helpful
Take this feedback and implement nurses thought and concerns in actual hospital setting to give sense of purpose and meaningfulness. RNs are not being ignored and their opinion matters
Request audit from nurse managers and assistant managers to help gauge impact of teaching and medication errors.
