Scenario for Milestone Assignment: Scenario is clear and concise, including a disease process, diagnosis, OR identify a patient with a desire to maintain good health and prevent illness. Include the nurse’s assessment of learning needs and readiness to learn.50 points | Patient Joe Smith is 40 years old and is slightly overweight. He came in for his yearly physical and to check on his blood pressure. He was diagnosed with pre-HTN at his last visit with it being 140/90. He was told to start monitoring his diet and exercise. He was also told to get a blood pressure monitor so he can do at home blood pressure readings. He has no other comorbidities, and no significant family history. Mr. Smith has a very stressful job as an accountant where he works almost 50 hours every week. He rarely takes his lunch break and usually just snacks on what ever he can grab quickly. By the time he gets home he is starving and tired. He grabs fast food on the way home because he is to tired to cook. Right after he eats, he goes right to bed. Nursing Assessment:Mr. Smith asked for help in controlling his blood pressure and stress. He is currently no on any medication and would like to keep it that way.Mr. Smith needs to reduce his stress which may be a factor to his HTN. Also, he needs to be educated on proper nutrition and preparing foods to take with him to work so he can make better diet choices. It would be beneficial for him to speak to the nutritionist to develop a meal plan that has a low sodium and include more fruits and vegetables. An exercise regimen also needs to be incorporated into his day since he is sedentary at work all day then at home. Breaking down 1 hour of exercise into two 30-minute increments or four 15-minute increments throughout the day can help get him to start becoming more active. |
