A large number of health care organizations are doing efforts for providing quality health services and ensuring an effective continuum of care. However, hospitals working at the national and international levels are the key pillars of health care systems. Doctors and nurses play a central role in these hospitals. I interviewed with one of my nurse colleagues in American central hospital. She was performing the duties of head nurse in the general ward of the hospital. She has to look after the patients in the general ward and has to keep a record of routine check-ups and medication of the patients. During the interview, we discussed several issues that are faced by hospitals on routine bases and then we came to know that one of the major issues hospitals are facing today is medication error. Medication error is the result of inappropriate communication between medical staff, especially nurses and may lead to lethal effects on patients.  Hospitals have adopted several practices for controlling these medication errors. Moreover, I came to know that hospital’s policy is strongly based upon interdisciplinary collaboration for ensuring better health services. Hospitals take part with other organizations and medical units. 

Issue Identification

While interviewing with my colleague working as a nurse in the hospital, I came to know that medication error has become a major problem in medical settings. Medication error is strongly linked with a lack of communication between health care professionals and this communication gap can be efficiently improved by adopting an interdisciplinary approach (Manias, 2018).

Change Theories That Could Lead to an Interdisciplinary Solution

The nursing sector is continuously striving for the provision of quality health care services to patients across the globe. They adopt and practice multiple strategies and treatment methods for curing a particular disease. Moreover, they are making progress at every moment, at every stage of their career. However, sometimes a change is required in their behavior, in their treatment method. Hence, change is the basic phenomenon of survival, the same is the case in the nursing sector. Lewin’s Change theories is applied in nursing for curing new strains of disease or to tackle old strains more effectively. Similarly, change theories may be applied for improving an interdisciplinary collaboration for reducing medication errors in nursing (Cleary et al, 2019).  Several studies have reported interventions and change theories in medical settings for reducing medication errors at different levels (Manias et al, 2020). Hence interdisciplinary collaboration has been used effectively for reducing medication errors by adopting different approaches such as reducing communication gaps using protocols and medication guidelines, reviewing the activities of nurses in promoting guidance about accurate medication, involvement of pharmacists in reducing medication errors, educating the nurses by conducting seminars and conferences, etc. For the investigation of medication errors causes and management in ICU, Lewin change theory is widely applicable. They used a qualitative study based upon human factor analysis. A large number of medical professionals and nurses participated in that analysis and they concluded that if prescription of a medicine is proper then medication errors can be reduced significantly (Sutherland et al, 2019).

A leader drives a sector with confidence, patience, and care. He devises new plans and strategies for achieving the objectives of the organization effectively. Thus, in the nursing sector, leadership qualities play an important role in providing the effective continuum of care.  Nurses should encourage and acknowledge the involvement of people working in other dimensions of medical settings. Moreover, they should make collaborative efforts to diagnose and resolve a problem in healthcare settings. For instance, pharmacists should be involved in finding the treatment of a particular disease. Nurses play a central role so their cooperation and good leadership skills can be effective in providing quality health services to the patients. 

Collaboration Approaches for Interdisciplinary Teams

A collaborative approach has been deployed for controlling and reducing medication errors.  Let us discuss an example of a collaborative approach where a committee was formed for analyzing the effectiveness of the collaborative approach and efforts for reducing medication errors in hospitals. An interdisciplinary team was organized containing medical staff and some other frontline workers.  These team members were involved in identifying medication errors in different hospitals and educating the medical staff and nurses regarding medication errors and their control. Moreover, the team extensively provided the medication guidelines and standard protocols for treatments. Resultantly, medication errors were reduced significantly by adopting this collaborative and interdisciplinary approach (Hanifin & Zielenski, 2020).  People are often visiting the clinics at a high rate and during each visit they interact with a large number of people including nurses, technicians, other patients, and doctors. Patients have to go for multiple options if the disease in not properly cured. Hence, interdisciplinary collaboration plays an important role in this regard. A lack of interdisciplinary collaboration and teamwork has become a serious challenge in healthcare settings. The effective continuum of care requires the interdisciplinary collaboration at every stage involving medical professional, social, cultural and professional barriers.  Healthcare professionals should share their skills and knowledge with other people so that exchange of ideas and expertise may lead to the fulfillment of basic goals of an effective coordination control and continuum of care.
