Interprofessional Collaboration
After discussing different topics for potentially developing new practice approaches, this week my preceptor and I discussed the selection of my change project topic and she assisted me with developing a PICOT question. After a long conversation over phone call, we both agreed on the topic of “Evidence-based practice fall intervention strategies in the hospital setting”. After selecting my topic, we discussed how Inter-professional collaboration is needed in order to properly initiate evidence-based practices fall intervention strategies in the hospital setting. We spoke on how it would take a team effort in order to increase the evidence-based practices fall intervention strategies in the hospital setting. We discussed about a 10-step protocol including screening for falls, comprehensive fall assessment, gait and balance screening when necessary, and an individualized fall intervention program addressing specific fall risks. We both agreed that keeping the patient’s personal possessions within patient safe reach, having sturdy handrails in patient bathrooms, room, and hallway, Placing the hospital bed in low position when a patient is resting in bed are some basic and main protocols for fall patients’ interventions
